Damages of the tibial post in constrained total knee prostheses in the early postoperative course a scanning electron microscopic study of polyethyle - June 11, 2008

Background: Investigation of the risk of fracture of the polyethylene (PE) inlay in constrained total knee prostheses. Methods: Three unused and seven polyethylene inlays that had been implanted in a patient's knee for an average of 25.4 months (min 1.1 months, max 50.2 months) were investigated using scanning electron microscopy (SEM). All inlays were of the same type and size (Genesis II constrained, Smith & Nephew). The PE surface at the transition from the plateau to the post was...

Biomechanical comparison of lumbar spine instability between laminectomy and bilateral laminotomy for spinal stenosis syndrome an experimental study - June 11, 2008

Background: The association of lumbar spine instability between laminectomy and laminotomy has been clinically studied, but the corresponding in vitro biomechanical studies have not been reported. We investigated the hypothesis that the integrity of the posterior complex (spinous process-interspinous ligament-spinous process) plays an important role on the postoperative spinal stability in decompressive surgery. Methods: Eight porcine lumbar spine specimens were studied. Each specimen was...

Differences across health care systems in outcome and cost-utility of surgical and conservative treatment of chronic low back pain: a study protocol - June 6, 2008

Background: There is little evidence on differences across health care systems in choice and outcome of the treatment of chronic low back pain (CLBP) with spinal surgery and conservative treatment as the main options. At least six randomised controlled trials comparing these two options have been performed; they show conflicting results without clear-cut evidence for superior effectiveness of any of the evaluated interventions and could not address whether treatment effect varied across patient.
